This incident turned out to be caused by an instrumentation failure. No patients have been impacted.

This incident has been resolved. Both clinicians and patients should be able to access the app without any issues.

We have identified the issue and we continue working on a fix for this incident.

We have worked with our SMS provider to roll out a fix and we are currently monitoring the results.

We have identified an issue with our SMS provider preventing all messages from being sent at the moment. We are working with their team to re-establish the service.
